A Patient Care Coordinators assess, plan, and advocate for the patients’ health needs. As trained health professionals, they monitor and coordinate patients’ treatment plans, guide them to understand the status of their health, and check in on their progress.

Job Responsibilities

• Demonstrates the ability to provide information in a way that is helpful and useful to the patient and all medical professionals providing them with care.
• Ensures a safe working environment through compliance with Hospital Policy and Procedure regarding safety, infection and security.
• Promotes incident, customer complaints reporting to improve patient and family care.
• Demonstrates skills in assessment of patient, family’s physical and psychological needs.

Job Requirements

• High School Diploma.
• Nursing Bachelor
• 4 Years experience in oncology medicals.
• Advanced Computer Skills in Windows, MS Office, Excel, Power Point, Internet and Email.
